ACCESS Economic Services 954-375-3338 Determines eligibility and ongoing case management of Food Assistance (food stamps), Temporary Cash Assistance and Medicaid for families with children or older adults, blind or disabled adults, based on state and federal guidelines.   • Social Security Cards for all members of the household on the application
  • Alien Status (I.D. Card) of all members of a household who are not U.S. citizens
  • Proof of household size and composition (# in the family, how many boys/girls, ages)
  • Resources (bank accounts, car registrations, etc.)
  • Proof of any and all income to all household members - loss or lack of income must also be proven. Provide at least 8 weeks proof of current income from employment
